Eli Lilly's orforglipron is the most exciting development in my opinion. It's an oral GLP-1 agonist that's NOT a peptide — it's a small molecule. This means no injections, no cold chain, and potentially much lower manufacturing costs. Phase 2 showed ~15% weight loss. Phase 3 is underway. Could this replace injectable GLP-1s entirely?

As someone with needle phobia, this is the drug I've been waiting for. The convenience of a daily pill with GLP-1 efficacy would be transformative. The question is whether the side effect profile is as manageable as the injectables.
The efficacy is slightly lower than injectable semaglutide (~15% vs ~15-18%) but the convenience factor is massive. Lilly is positioning this as the mass-market GLP-1. Expect it to be a top-selling drug within 2 years of launch.
