Gray-Market Retatrutide Users Report Increased Appetite Most Often, Penn AI Analysis Finds

Topics: retatrutide gray market, gray market peptides, retatrutide side effects, JAMA Network Open, peptide safety, unapproved GLP-1, Reddit drug reports

A University of Pennsylvania team has produced the first systematic look at what gray-market retatrutide users report experiencing, using artificial intelligence to read roughly 150,000 Reddit posts and comments from about 40,000 users.

What Happened

The analysis, published in JAMA Network Open, identified approximately 14,000 users who said they had personally taken an unapproved version of retatrutide, a triple-agonist compound that has completed neither late-stage trials nor regulatory review. Researchers narrowed that to 7,823 users who described at least one symptom mappable to standardized medical terminology, after excluding expected effects such as weight loss.

The most frequently reported classifiable symptom was increased appetite — a result at odds with retatrutide's Phase 2 trial record, where nausea and other gastrointestinal complaints led.

Compared with the same lab's earlier analysis of semaglutide and tirzepatide discussions, retatrutide posts carried proportionally more reports of appetite changes, sleep disturbance, elevated heart rate and muscle-related complaints, and fewer reports of nausea and digestive upset.

"We can't say that retatrutide is causing these symptoms, or even that Reddit users are actually taking genuine retatrutide, since the drug is not legally available, and gray market formulations vary," said co-author Lyle Ungar, a professor in Computer and Information Science.

Who Is Affected

  • Gray-market buyers, who cannot independently confirm the identity, purity or composition of what they inject.
  • Clinicians, who may take histories from patients using products that never appear in a pharmacy record.
  • Testing labs and harm-reduction groups, currently the only parties generating compositional data.
  • Research suppliers, whose credibility is measured against a market they do not control.

Timeline

  • October 15, 2026 — Penn announces the findings from its Computational Social Listening Lab.
  • October 2026 — the study publishes in JAMA Network Open, with Neil Sehgal as first author and Sharath Chandra Guntuku as senior author.
  • Earlier work by the same team — an analysis of more than 400,000 posts about semaglutide and tirzepatide surfaced menstrual irregularities; another group later found disproportionate reports of similar changes in the FDA's adverse-event database.
  • Ongoing — retatrutide remains in Phase 3 development, so every user in this dataset was self-administering an unapproved product.

What This Means for Researchers

The study is a hypothesis generator, not a safety verdict. It shows that AI-assisted reading of patient forums can surface symptom patterns trial reporting does not capture, and it documents a population sitting entirely outside conventional pharmacovigilance — no sponsor, no protocol, no mandatory adverse-event channel.

The authors are explicit about the limits: people willing to buy an unapproved injectable online may differ from trial participants, may be running several experimental compounds at once, and cannot verify what they received.

How to Verify Your Peptides

For anyone handling research materials, compositional testing is the only way to know what is in a vial:

Safe Alternatives

Approved incretin therapies exist, and their safety records were built in trials with structured adverse-event reporting. Start with our compound references for semaglutide and tirzepatide, browse the peptide library, and review vendor profiles where third-party testing documentation is public.
