A1c creeping up while CGM average stays flat?

T2D for 4 years. A1c went 8.1 to 6.6 by month 8, then drifted to 6.7 at month 12 and 6.8 at month 15. My CGM 14-day average is 118, fasting 112-126, and I don't see many spikes. Weight is down 18 lb but has stalled since March. Same lab each time. What am I missing? Iron? Sleep? RBC turnover? Anyone had A1c drift up while CGM stayed flat?

That CGM average of 118 and A1c 6.8 don't line up for me either. 118 usually maps closer to 5.7-5.8. Did the lab use a different assay than before?

Iron can skew it too. My ferritin was 11 and my A1c read high until that was fixed. GP never mentioned it.

Sleep is my missing piece. Bad sleep week equals fasting in the 120s. CGM average stays okay but mornings look ugly.

If CGM average is 118, where are the highs hiding? A1c 6.8 implies more time above 140 than you'd think.

Could be post-meal spikes you're not seeing if you scan less? I found I was 180 after rice even when fasting was fine.

I had the opposite. CGM said 130 average, lab A1c 6.4. Turned out my sensor ran high in the last two days.

Mine stalled around month 14 too. Weight loss stopped and A1c crept 6.5 to 6.7. Added 20 min walks after dinner and it moved a bit.

The weight plateau is interesting. Are you eating less than before because appetite is still low, or has it come back? That changed my evening grazing.

What stood out to me is the discordance. A1c reflects about 3 months of glucose, but CGM only shows what it sees. If you're not wearing it consistently, or if you're mostly looking at fasting, the average can look fine while post-meal numbers are higher. I'd compare a 14-day CGM report with time-in-range and lab A1c before assuming insulin resistance changed. Also, any recent illness, steroids, or less activity? Those can shift things without the CGM average moving much. The mismatch is why I ask for the AGP report, not just the daily graph.

Your iron comment made me check old labs. My A1c was 6.9 when ferritin was low, then 6.5 after it normalizde. Wild.
