Injection site reactions — what's worth documenting, and when do you raise it?

For those who track this, what do you record — for me at least. I've noticed some welts at the site lately, so rather than guess I've started logging each occurrence — date, location, size, how long it lasted — to see if there's a pattern. Would love to hear any thoughts on at what point would you flag a recurring reaction to a clinician rather than just note it and move on.
