Lilly's EloraTZP Delivers 23.3% Weight Loss in Phase 2b, Advancing to Phase 3

What Happened

Eli Lilly has introduced a new name into the weight-loss lexicon: EloraTZP, a combination of the selective amylin agonist eloralintide and Lilly's dual GIP/GLP-1 receptor agonist tirzepatide. Presented at the European Association for the Study of Diabetes (EASD) annual meeting in Milan, the Phase 2b trial enrolled 367 adults with obesity or overweight and type 2 diabetes.

At 48 weeks, the highest combination dose produced average weight loss of 23.3%, compared with 14.8% for tirzepatide alone and 3% for placebo. Eloralintide on its own cut weight by up to an average of 12.3% in the same trial. Blood-sugar control improved in parallel: A1C, a measure of average blood glucose over the preceding two to three months, fell by an average of 2.9 percentage points on the top combination dose, versus 2.4 for tirzepatide alone and 0.3 for placebo. Every dose combination tested met the trial's primary and secondary endpoints, and Lilly said it will launch Phase 3 trials before the end of the year.

First, the efficacy numbers use the efficacy estimand — they reflect outcomes for participants regardless of discontinuation, a framing choice that flatters the topline when tolerability is rocky. Second, tolerability is the open question: discontinuation due to adverse events ranged from 10.8% to 27% across combination dose arms, versus 2.9% for tirzepatide alone. Lilly executives attribute this to the short 48-week design and the simultaneous titration of two drugs, and say the Phase 3 study will run longer with an optimized escalation schedule.

The strategic lesson is pharmacological: pairing amylin agonism with incretin agonism produced effects neither class achieves alone — evidence that rational peptide combinations, rather than single-molecule escalation, may be the next frontier of metabolic drug development.
