Week 32 in survodutide trial: appetite back, waist down 9 cm

Something that comes up in blinded phase 3 obesity trials around week 32: the first 16 weeks appetite is basically gone, then hunger creeps back to roughly 70% of baseline — even while the tape measure and the labs keep moving the right way. Waist down 9 cm. Fasting glucose 92, down from 104. A1c 5.2, down from 5.7. Meanwhile the scale sits at -11.4 kg for three weeks without budging.

The part people forget is that the appetite rebound can show up before the next scheduled visit, and from the inside there's no way to tell whether it's the drug fading, a dose question, or just the maintenance phase settling in. Common surprise: trial coordinators mostly log it and say keep going — that's their job, not their call, so don't expect much interpretation there.

Worth knowing before comparing options or comparing notes: stalls at the same time as hunger returning is a pattern, not necessarily a problem. If that's happening in a blinded trial, it's a genuinely useful thing to compare against other people's timelines.

Same thing happened to me around week 30. Hunger definitely came back, but my clothes kept fitting looser the whole time. Scale didn't move for 26 days, then dropped 1.3 kg overnight.

I'm not in your exact trial, but I follow the phase 3 reports. Appetite return befoer a scheduled titration isn't rare; the 9 cm waist drop with flat weight sounds like body comp shift. Did your site measure hips too? Mine only does waist, and I keep wishing they tracked more. I also ask because my own waist moved before my fasting glucose did, which surprised me.

26 days is rough. Did you change protein or steps during that stall? I'm trying to figure out if my week 28 plateau was just water.

A 26-day stall is interesting. From what I've seen in tracking threads, glucose tends to move first and weight lags behind by like 6 weeks. Fasting glucose 92 and A1c 5.2 are solid though.

Hip measurement matters. My waist stalled but hips dropped 5 cm. Site nurse said it's common in the trial but wouldn't say more.

Blinded trials mess with your head, especially when the scale stalls.

My scale sat on the exact same number for five straight weeks around month 7, and the weird part was my waist kept shrinking — about 2.5 cm during that dead stretch, which is basically what convinced me the scale is a liar with a delay. What actually unstuck things had nothing to do with appetite: I was sleeping 5.5 hours most nights and holding water like a sponge, and once I got back to 7+ it dropped 1.8 kg in about 10 days with zero change in how much I was eating, so I'd check sleep and sodium before assuming your body's fighting you. I'd also gently push back on calling 70% hunger a rebound when your waist and glucose are both still moving the right way — mine got loudest when I was under-eating protein, and a real 35-40 g at breakfast (eggs, Greek yogurt, whatever's easy) quieted the afternoon grazing way more than waiting it out did. Are you measuring your waist at the same time of day with the same tape tension? Mine swings a full 2 cm just from bloat and last night's dinner.

Week 28 here and my hunger came roaring back for about 10 days, mostly 8-10pm, but the weird part was my jeans got looser during that stretch—down one belt hole and 2 cm on the tape—so I stopped treating it as a sign it was failing and started treating it like a timing problem. What helped was moving my biggest protein/fiber hit to lunch and doing a 15-minute walk after dinner; if I skip the walk I’m hunting the pantry by 10. Is yours true stomach hunger or more mental/evening habit? That would change what I’d tweak next.

Week 32 here too, and my waist tape has been way more honest than the scale — but only after I stopped eyeballing it. I was measuring over whatever shirt I had on and getting anywhere from 3 to 7 cm down depending on the day; switched to bare skin, same landmark at the navel, first thing in the morning after the bathroom, and it settled into a real 8.5 cm. Weird reversal though: my worst tape week (up 1.5 cm) was four bad-sleep nghts plus two restaurant dinners, and the scale actually dropped that week. One number went up, the other went down, and neither was lying. So I'm curious whether your 9 cm is bare skin at the same spot every time, and whether anything in your closet has gotten roomier while the number sat there — that's been my tiebreaker for the weeks the scale gets boring.

The tape measure is the only one not lying to me lately — mine's hiding in the snack drawer.

I noticed a similar shift where hunger returned but meals still ended sooner. What helped me was logging pre-meal hunger and post-meal fullness on a 1–10 scale, not just calories. My pre-meal hunger crept back up, but fullness stayed higher and portions stayed smaller. I also measure my waist first thing, same landmarks, after exhaling, and only compare weekly averages. Did you notice whether your fullness signals changed too, or mostly the

9 cm is huge — celebrate that, plus those labs. Something new I started around that point: a 7-day “energy and stairs” note in my phone. Scale stalled at -11 for me too, but I could take the stairs without pausing and sleep through the night. That kept me from spiraling. Curious: are you tracking any habit or recovery markers besides the tape? I’m trying to decide whether to add waist-to-height or just keep it simple.

Tbh in the trials, appetite ratings often stop matching the scale around that point. What helped me was separating “hunger” from “head hunger”: I logged 1–10 before meals and noted triggers. Mine was 7/10 at 5pm but zero actual stomach growl; a 30g protein lunch + 10 min walk after dinner cut the background noise. Also added 2x/week resistance bands to protect lean mass while waist dropped. Not saying it caused anything, just my n=1. Did yours spike at a specific time, or is it more constant?

appetite respawned but the tape measure still carried the match, grats on 9cm!

I have found waist-to-height ratio more useful than the raw centimeter count once the scale gets less dramatic. Mine moved from 0.58 to 0.52 over a six-month stretch, even with a few flat weeks. I take it once weekly, same morning, before breakfast, at the navel, and average three readings. It gave me a steadier signal than daily hunger or weight. Do you know if your trial is also tracking that ratio, or mainly waist circumference? That might be worth asking at your next check-in.

Curious whether your lifts have held through this stretch. I log waist at three spots — navel, narrowest, and 2 cm above the hip bone — and average them, same morning, fasted, after exhale. When hunger came back

My scale pouts while my tape measure celebrates — you're right on trend, friend.
