Tracking 6 months of telehealth consult/lab fees — is this creep normal?

I’ve been tracking my telehealth provider costs since January: $149 initial consult, $79 follow-up, $62 lab draw, $199 annual membership. That’s $489 before any medication is even mentioned. In February they added a $25 “chart review” line, and April follow-up went to $89. I asked billing and got a vague “administrative adjustment” answer. Is everyone seeing this creep, or is it regional? I’m trying to build a simple spreadsheet of consult vs lab vs membership fees. If you’ve kept numbers, what changed for you this year?

My clinic did the same but called it a platform fee. Consult went $125 to $150 in March. Lab draw still $0 if I use their preferred lab, but out-of-network is $78. I’d split those line items in your sheet.

Regional maybe. I’m in teh Southeast and follow-ups are still $65. No membership, but they require a $35 “records” fee once a year.

The chart review fee would make me ask for an itemized superbill. Not medical advice, just billing. Does yours list CPT codes or just vague admin labels?

Mine lists codes. Setup code showed up twice in Jan.

Duplicate setup codes are a pain. I gave up and just budget $600/year, which is wild.

The regional piece is real. I moved from the Midwest to the West and my follow-up jumped from $65 to $105 at a similar clinic. The pricier one does include portal replies and same-week scheduling, though. I still keep a sheet because the first clinic added a $30 “non-clinical review” fee after I’d paid for a year. That felt less like creep and more like bait-and-switch. For me, total annual cost matters more than the consult headline. I also separate one-time onboarding from recurring fees so the trend is clearer.

Same-day portal replies would justify some of it. My current clinic says 72 hours but often hits 5 days, which is why I’m watching fees closely.

Total annual cost is the only number I trust.

The pricing never had a single line item go up — the creep was cadence. Back in August it was $99 initial and $45 follow-ups every 8 weeks, so the non-medical spend for the year penciled out around $370. Sometime in the spring they "recommended" moving to every 4 weeks for "better monitoring," which nobody forced, but sure, and suddenly the annual math is a different animal.

For what it's worth, i’d ask for an itemized superbill with CPT codes, not just a balance summary. That’s how I caught a “chart review” fee that was actually bundled into the follow-up code at another clinic. Also check whether your annual membership auto-renews and what pausing or canceling does to any pending lab orders. Did your statements show codes, or just vague line names like “administrative adjustment”? Without codes, it’s really hard to tell if it’s a new service or just repackaging.

Not a lawyer, but if you’re self-pay, the No Surprises Act gives you the right to a Good Faith Estimate before care. I started asking for one each quarter; it forced my clinic to put the membership, chart review, and follow-up cadence in writing. My big surprise was that lab draw and lab processing were separate—$62 only covered the draw, not the panel. Do your statements split those? Also, are these card-on-file auto-charges? That’s what made mine feel creepier than the amounts.

I’ve had luck keeping a dated screenshot of the price shown before each booking, plus the confirmation email. When a surprise “admin” fee appeared, I asked them to point to the exact date I agreed to it—they credited it back. Does your provider post a self-pay fee schedule anywhere, or is it only revealed at checkout? Also worth checking if

Night shifter here. The $25 “chart review” is the tell. Ask billing for the written self-pay fee schedule and the policy that says chart review is separately billable. Not an itemized bill—the actual price list. If they won’t send it, that’s not an admin adjustment, that’s made-up. Also check the membership auto-renew terms and cancel-by date. I put that in my phone the day I sign up. One missed window and you’re stuck. Prices creep. Terms creep harder.

For what it's worth, not just you — I started screenshotting every booking total after a surprise fee popped up.
