Genuine question — is there any reason to cycle tirz rather than take it continuously? I've seen people mention taking breaks to 'reset receptor sensitivity' but I'm not sure how much of that is broscience. My only break was forced (supply gap) and the food noise came roaring back within days. What's the actual thinking here?

I had the same concern. My doctor said the same thing — it's the rapid weight loss, not the drug. Mine stopped after about 24 months once my weight stabilized.
My A1C went from 9.3 to 6.0 in 6 months on tirz. My endo was shocked. The blood sugar control is honestly more important than the weight loss for me.
I've been at 7.5mg for 18 months now. The jump from 5 to 7.5 was noticeable — more appetite suppression but also more nausea for the first two weeks. It settled down after that. I'd say go for it if you're stalling at the current dose.
