Orforglipron's ACHIEVE-4 Cardiovascular Data Raises the Safety Bar for Oral GLP-1s

Topics: orforglipron, ACHIEVE-4, oral GLP-1, MACE, cardiovascular safety, insulin glargine, type 2 diabetes, Eli Lilly

Eli Lilly's experimental oral GLP-1 receptor agonist orforglipron has cleared a major cardiovascular hurdle, reporting noninferiority to insulin glargine on major adverse cardiovascular events (MACE) in its largest and longest type 2 diabetes trial to date. The ACHIEVE-4 results, presented at a European diabetes congress in October 2026, mark a key de-risking milestone for the small-molecule pill and the oral incretin class behind it.

What Happened

Lilly reported that orforglipron met its primary cardiovascular safety endpoint in the ACHIEVE-4 Phase 3 trial, which randomized 2,749 adults with type 2 diabetes at increased cardiovascular risk to once-daily oral orforglipron or titrated injectable insulin glargine. Over a median follow-up of two years:

  • MACE-4 events occurred in 4.2% of orforglipron patients versus 5.0% on insulin glargine (hazard ratio 0.84, 95% CI 0.59–1.20), meeting the noninferiority margin
  • Clinically significant or severe hypoglycemia was roughly three times less frequent with orforglipron: 6.8% versus 19.2% with insulin
  • At 52 weeks, orforglipron lowered A1C by 1.6% from a mean baseline of 8.22% and produced 10.4% weight loss, versus a 1.0% A1C reduction and weight gain on glargine

Who Is Affected

  • Adults with type 2 diabetes and elevated cardiovascular risk — 85.9% of participants carried established cardiovascular disease and 37.6% had chronic kidney disease, a high-risk population
  • Lilly and its development partners — the result removes the largest remaining safety question over the pill's approval path
  • Competitors running oral GLP-1 programs — the data sets the evidentiary bar every later-stage oral program must now clear
  • Patients on insulin therapy — the active-comparator design gives clinicians a like-for-like picture of switching from glargine

Timeline

  • October 2026 — ACHIEVE-4 cardiovascular safety results presented at a major European diabetes congress
  • 2026 — Regulatory filings for orforglipron are expected to incorporate the cardiovascular data package
  • Ahead — A dedicated cardiovascular outcomes trial remains the standard for establishing superiority, which injectable GLP-1s have already achieved

What This Means for Researchers

The noninferiority result is meaningful but should not be overstated. Noninferiority to an active comparator is a different evidentiary statement than cardiovascular superiority over placebo, which landmark injectable trials such as SELECT and LEADER demonstrated. Orforglipron now has proven cardiovascular safety in a high-risk population; it does not yet have proven cardiovascular benefit at that standard. Pulse rate rose with orforglipron and stayed elevated at week 104 (mean +4.0 beats per minute), a signal clinicians will watch. The result still reshapes expectations: any oral incretin program entering late-stage development will be measured against this benchmark, and generic 'safe' claims for unapproved oral peptide products carry even less credibility than before.

How to Verify Your Peptides

Oral GLP-1 pills like orforglipron are investigational and unapproved anywhere; anything sold online today as 'oral GLP-1' or orforglipron itself is counterfeit by definition. Independent verification remains the only protection:
