The first real-world study to track patients who moved from weight-loss injections to the oral semaglutide pill found they kept losing weight after the switch, an early signal that a tablet can hold the line where many people otherwise regain.
What Happened
The OCTANE study followed 200 people who switched from injectable GLP-1 medicines, including Wegovy and tirzepatide-based Zepbound, to the daily oral semaglutide tablet. Outside the controls of a clinical trial, participants lost an average of 4.0 kg (8.8 lbs), about 4.1 percent of body weight, over three months, from an average starting weight of roughly 100 kg.
Nearly 41 percent lost at least 5 percent of their body weight, and the share classified as clinically obese fell from 87.1 percent to 65.5 percent. The findings were presented at the European Association for the Study of Diabetes annual meeting in Milan.
Timeline
- September 30, 2026 — OCTANE results presented at the EASD annual meeting in Milan
- Three-month follow-up — Average 4.1 percent further body-weight loss measured after the switch
- Context — Oral semaglutide set the record for the fastest weight-loss drug launch in history earlier this year
Who Is Affected
- Injection-averse patients — People who stopped injections over needles, cost, or convenience and previously faced weight regain
- Oral GLP-1 buyers — Demand for the tablet is already drawing gray-market copies sold without approval
- Clinicians — A real-world data point for maintenance strategies after injectable therapy
- Competitors — Novo Nordisk defends its lead in oral obesity treatment against Eli Lilly's pipeline
What This Means for Researchers
- Real-world evidence, not a trial. OCTANE is a company-presented observational study of 200 switchers, with no randomized control group; it shows plausibility, not proof
- The regain question is the real target. Injectable GLP-1s deliver dramatic loss, but discontinuation typically reverses it; a tolerable oral maintenance option changes the long-term curve
- Adherence held up. Most participants took the tablet consistently, the finding its sponsor flagged as the most important
- Three months is short. Whether the pill sustains weight loss over years remains unanswered
