Keto flipped my lipids, now what?

I’m 34, an ER nurse with two kids, so meals are chaotic. Went low-carb in January for energy and to stop the 3 p.m. crash. Energy’s better, lifting is consistent again, but my last lipid panel weirded me out—trigs improved while LDL jumped. I’m not sure if I should retest ApoB now or wait until my weight levels off. What marker actually made you change course?

Welcome. My LDL jumped early while I was losing. I retested at 6 weeks after weight stabilized and it settled.

Trigs 210 to 95 is the real headline here.

Agree on stable weight being the goal. But the charts can look wild — 130 up to 175, then back down to 135 once the losing phase flattens out. Number settles eventually, just never in a straight line.

What did your ApoB do? Mine tracked LDL closely, but trigs/HDL ratio was what finally made me stop obsessing over total LDL. I also lift 3x/week. Not a doctor, just my n=1.

The trigs/HDL ratio thing is underrated. Mine went from 4.6 to 1.7 and I stopped panicking about LDL.

If lifting is new, my LDL was higher during the first months too. It calmed down after training got consistent.

I'm 39M and had almost the same numbers: trigs 230 to 110, HDL 42 to 55, LDL 115 to 190 at week 10. I waited until week 20 when weight was stable. LDL came down to 150, ApoB 95. What changed my direction was more monounsaturated fat and soluble fiber, not cutting saturated fat to zero. Your A1c move is solid. What's your weekly weight loss rate right now?

I retested ApoB at stable weight too. Mine lagged LDL by about 6 weeks.

Disagree on waiting. I tested ApoB at 12 weeks while still losing and it gave me useful data.

That's fair, but data while losing can be noisy. I did both: ApoB at week 8 and week 16. Week 8 was 110, week 16 was 95. I don't regret the early test, but I wouldn't have changed much based on it. The bigger signal for me was waist and fasting glucose. If you're data-driven, maybe track ApoB, trigs/HDL, and weight trend separately so you don't mix fat-loss effects with diet effects. I also wish I'd measured ApoB before starting, because a baseline would've made the week 8 number less confusing. What are you using to log? I just use a spreadsheet and a tape measure.

A1c 5.7 to 5.5 in 8 weeks is no joke. Nice work.
