Lilly's Foundayo Meets Heart-Safety Goals in Largest Type 2 Diabetes Trial

What Happened

Eli Lilly has disclosed the fullest dataset yet from ACHIEVE-4, the largest and longest Phase 3 study of its oral GLP-1 receptor agonist Foundayo (orforglipron) in adults with type 2 diabetes. The trial tested the once-daily pill against titrated insulin glargine in adults with type 2 diabetes and obesity or overweight who carried elevated cardiovascular risk.

Foundayo met its primary endpoint, demonstrating noninferior risk of major adverse cardiovascular events (MACE-4) versus insulin glargine — a 16% lower observed risk of the four-way composite of cardiovascular death, heart attack, stroke or hospitalization for sudden unstable chest pain.

Prespecified analyses went further:

  • A 23% lower risk of MACE-3, the composite of cardiovascular death, heart attack or stroke
  • A 53% lower risk of cardiovascular death and a 57% lower risk of all-cause mortality compared with insulin glargine
  • An A1C reduction of 1.6 percentage points from a baseline of 8.22%, versus a 1.0-point reduction on insulin
  • Average weight loss of 8.8% — roughly 18 pounds — versus a 1.7% weight gain on insulin, with more than a third of Foundayo patients losing at least 10% of body weight against 3% of insulin patients
  • Improvements in waist circumference, systolic blood pressure and non-HDL cholesterol, plus a slower observed decline in kidney function

In a separate post-hoc analysis of the ATTAIN-1 study, the same oral agent was associated with an estimated 57% reduction in predicted type 2 diabetes risk and an 18% reduction in predicted cardiovascular disease risk versus placebo in adults with obesity-related comorbidities.

First, ACHIEVE-4 is the most extensive cardiovascular dataset yet published for a non-peptide, orally active GLP-1 agonist — a small molecule engineered for oral bioavailability rather than a modified injectable peptide. Noninferiority against an active insulin comparator in a high-risk population is a meaningful benchmark for the entire oral-agonist class.

Second, the mortality findings should be read as hypothesis-generating: they were prespecified but sit outside the trial's primary hierarchy, and ATTAIN-OUTCOMES is the study designed to answer the outcomes question definitively.
